Old Age Pensions Schemes
A person is eligible for the grand of old age pensions schemes who is aged 60 years or more, is a domicile of Haryana State and is not receiving of Rs. 300/- P.M. or more. If both husband and wife qualify for pensions schemes both of them will be entitled to receive the pensions schemes separately. If an exservice man is getting military person of more than Rs. 300/- p.m., his wife will be entitled for old age pensions schemes if she is other wise eligible. The following person shall be excluded for the grant of old age pensions schemes:-

1. Those who themselves or their children are sales tax assesses.
2. Those whose children are clam I/II gazetted offices in the service of Govt. or hold equivalent post in public sector under taking.
3. Those whose children are professional l.e. doctor lawyer, C.A., Dentist, Engineer, Architect, Contractor, etc.
4. Those whose children are income tax assesses.
5. Those who themselves/ their children are ex-siting members of M.P./M.l.A./Chairman of board/Corporation.

The scruting of eligible new beneficiaries are identified in every year as per the following schedule:-

1. Constitution of survey teams & munadi 1st to 31 October every year
2. Actual survey to identify new beneficiaries 1st November to 15th December
3. Finalization of objections/appeals if any by district committees
headed by deputy commissioners
16th December to 31st December
4. Submission of final list by DWSOs concerned to HQ 15th January {ensuing year}
5. Approval of final lists by the state government {to provide for sufficient
time to make a budget provision for the ensuing year}
By the 7th February
6. Date of start of payment of old age pensions schemes to new beneficiaries with effect from 1st April every year
Old age pensions schemes is disbursed to the eligible beneficaian in every month? At village level through parelayal and in urban area through were budie regularly.

Widow & Destitute Women Scheme

A woman in the age group of 18 years and above is eligible for the grant of pension. If she is a widow or unmarried or a married woman who has been depressed of the financial support from her husband be cause of his physical/mental in capacity or desertion by husband or any other reasons and her close relative such as parents, sons, are not supporting her and her own income from all sources in less than 10,000 (The thousand) per year. She is a domicile of Haryana and has been residing in Haryana State last 1 years at the time of submission of application. The amount of pension 350/- p.m. is disbursed to pensioner in every month. For identification of new eligible beneficiation in the rural areas the committee consisting of D.S.W. his representation circle revenue officer and medical officer and in urban areas members are secretary/E.O./M.C., D.S.W.O. or his representatives and medical officer.

The surpunch/Lamberdar/Municipal Councillor will attest the application form. Area Patwari will give income report of applicant and application form. After signatures of Lamberdar/suspanch/M.C., security committees and application form the applicant will submit the form along with attested Photostat ration card/voter card, attested Photostat death certificate and affidavit of re massage. The pension will be sanctioned from the month of submission of form.


Handicapped Pension

A person in the age group of 18 years and above shall be eligible for the grant of handicapped pension he/she is a domicile of Haryana State and has been residing in Haryana State his own and family income from all sources in less Rs. 10,000/- (Ten Thousand per year). He/She suffers from one or more of the following handicaps.

1. Total absence of sight.

2. A lose of sense of hearing.

3. Orthopedic handicap with permanent disability of 70% and above.

4. Mental retardation with i.q. not exceeding 50.

Those who have above disability they will get 300/- p.m. pension and those who Have 100/- (Hundred percent) disability they will get 600/- per month pension.
Application form will be attested by Lamberdar/Surpanch/M.C. and area patwari mentioned will report the income report in the form. After that security committee consisting of Doctor, Circle Revenue officer/secy. of M.C. and D.S.W.O. or his representative recommended for pension. The pension will be sanctioned from the month of submission of form in the office.


Financial assistance to destitute children
A child under twenty one years of age who has been depressed of parental support or care by season of death, continued absence from home of his father for the last 2 years or father/mother has been sentenced imprisonment for a period not less than one year or physical or mental incapacity of the parents and whose parent’s/guardian’s income does not exceed Rs. 10,000/- per annuam shall be eligible for Financial assistance.
The applicant shall be required to furnish proof of age in one of the following manner(1) school certificate, (2) Birth certificate issued by registrar birth & death. The Financial assistance shall be given at the rate of Rs. 100p.m. per child to meet the cost of maintence and education provided that at maintence and education provided that not more than two children in a family shall be eligible for financial assistance.
The application form is attested by Gazetted officer, income report is verified by area patwar/Tehsildar. The form will available free of cost from the D.S.W.O. office. The assistance is disbursed along with pension in every month.

Unemployment pensions schemes to the physically handicapped persons
All educated unemployed applicants shall eligible for the grant of unemployment pensions schemes who are physically handicapped in accordance with the criteria and are enrolled on the live register of employment exchange in Haryana. Those who are 70% and above handicapped the rate of pensions schemes as follows:-

1. Matric/Middle pass diploma holder Rs. 200/- p.m.
2. Graduate/Matric pass diploma holders Rs. 250/- p.m.
3. Post Graduate/Graduate diploma holder Rs. 300/- p.m.

Rates of unemployment pensions schemes to 100% handicapped would be as under:-

1. Matric/Middle pass diploma holder Rs. 1000 p.m.
2. Graduate/Matric pass diploma holder Rs. 1500p.m.
3. Post graduate/Graduate diploma holder Rs. 2000/- p.m.

Financial assistance will be given for a period of six year From the month of application form is received in the office. Handicapped person will required to give an affidavit at the beginning of every fi9nancial year that they continued to be unemployed. The unemployment pensions schemes shall be remitted to the beneficiaries as quarterly basis. Application form should be available free of cost is the D.S.W.O. office. Fresh applications form should be supported by the following documents:-

(1) Three specimen signatures duly attested by gazetted officer.
(2) Attested disability certificate.
(3) Attested educational certificates.
(4) Registration certificate from the unemployment office.
(5) Attested prescribed affidavit.


Scholarship to the Handicapped students
A disable students who is a domicile of Haryana, having disability 40% and above, getting education in the educational institutions may apply for the scholarship from I class to on wards whose family income from all somas not exceeding 4000/- p.m. for I to VIII class and rate of scholarship I to IV class Rs. 100 p.m., V to VIII class Rs. 120 p.m. and for IX to on wards class whose family incosme is not exceeding 6000/-p.m. rate of scholarship as follows IX to 10+2 125 p.m. graduation class Rs. 200 p.m. post graduate clame Rs. 300 p.m. An application form will be available from the office of D.S.W.O. and free of cost. The application form is duly filled by the institution every year. The application form shall be accompanied by disability certificate and income certificate by not below the range of Naib Tehsildar. If the student is failed in the class from the scholarship will not be admissible for the same class.

Dwarf of pensions schemes
Eligibility

The applicant shall be a domicile of Haryana has been residing of Haryana State for the last one year at the time of submission of application.

The age of the applicant shall not be less then 18 years.

The applicant must give a certificate from the Civil Surgeon is support of being drawer.
A male person of 3 feet 8 inch or less light and female with 3 feet 3 inch or less light(equilent to 70% handicapped ) would be entitled to monthly pensions schemes.

The applicant will apply in the prescribed application form which will be available form district Social welfare officer free of cost.

Each applicant will submit an applicant along with medical certificate to the District Social Welfare Officer. All fresh applicants /or claim of pensions schemes required would be remained and those found eligible during the month will be sanctioned and bought and roll from Ist of the month during which the same would be found eligible. The rate of pensions schemes shall be as decided by the Govt. from time to time. (At present 300/-p.m.)

Eunuch pensions schemes

In this scheme the applicant have a domicile of Haryana State and the age of applicant should be 18 years or above. Applicant must give a certificate from the Civil Sugreon is support of being enunch. The rate of pensions schemes shall be decided by the Govt. from time to time. (At present 300/- p.m.)
Self signed application form with ration card copy may submitted to the office of Distt. Social Welfare Officer.


National Benefit Scheme(N.F.B.S.)

Family under this scheme on the death of head of bread earmed of family under B.P.L., in age group 18 to 64 years, Govt. will be provided help of Rs. 10,000, application form of the scheme shall be available as the office of D.S.W.O. free of cost self signed application form with 2 photo, ration card copy, death certificate copy, no marriage affidavit, BPL list copy, attested by sarpunch/namberdar, be submitted to the consurned D.S.W.O. office with in one year of death.


Identity card to the handicapped person

Under this scheme the office of the District Social Welfare Officer shall be issued the identity card to the person having certificate 40% and above disability. Application form shall be available in the office of D.S.W.O. self signed application form must be attached with 2 photo, Ration card copy, blood group certificate and handicapped certificate issued by Civil Surgeon.


Miniority communities welfare scheme

The ministory of minorities affairs introduced a scheme of pre matric scholarship for student belonging to the minority communities notified by the contol Govt. which shall be implemented through the state Govt. with three type of scholor ship as below:-
1. Pre matric scholarship scheme.
2. Past matric scholarship Scheme.
3. Marit-cum-mea scholarship scheme.

Rajiv Gandhi Parivar Bima Yojna
Please refer to the subject cited above.
It is intimated that the Govt. has entered into MOU with Reliance General Insurance Company Ltd. For implementing Rajiv GHandhi Parivar Bima Yojna in Haryana State w.e.f. 28th march, 2008 for one year. The main terms & conditions of the scheme and of this MOU are as under:-

1 DEFINATION

(i) Insured/Beneficiary The insurane scheme shall cover all the persons in the age group of 18 to 60 years of all the families in Haryana whose names appear in the voter’s list of Haryana or who hold Ration Card issued by the Food and Supplies Department of Haryana except government employees and income tax payees subject to the conditiond that only one person in a family shall be given benefit of insurance under this scheme. Benefit of the scheme for death or permanent disability resulting from accident occurring even outside Haryana shall be extended under the scheme.
(ii) Categories excluded from the Scheme
(a) All income tax payees.
(b) All the employees of Government, and its Public Sector Undertakings, Co-operative Institutions, Universities, Government Aided Institutions etc. as they are already covered under Group Insurance Scheme.

2. CRITERIA FOR VERIFICATION OF INSURED/BENEFICIARY

(a) Name of the insured/beneficiary shall appear in ration card or voters list, and
(b) Certificate by the concerned Sarpanch or municipal Commissioner regarding age, status of the deceased/ insured and domicile of the deceased/insured.

3. COVERAGE

(i) Death/permanent total disability/disability due to accidents like:

(a) rail/road/air accidents;
(b) riot, strike and terrorism;
(C) snake bite, drowing;
(d) poisoning, electrocution;
(e) falling from height, collapse of house/building;
(f) fire, explosion, implosion;
(g) murder;
(h) attack by animals;
(i) stampede and suffocation etc.;
(j) frost bite, sun burn (loo), lightening;
(k) burn injuries;

(ii) Death/permanent total disability/disability due to occupational hazards like threshing machine/ industrial machine etc. and due to any other unnatural events except the following:

(A) from-
(a) war and allied perils;
(b) nuclear risks;
(c) self injury, suicide or attempted suicide, or

(B) whilst-
i. under the influence of alcoholic drinks or drugs;
ii. engaging in aviation activities other than traveling as a passenger;
iii. breaching any law with criminal intent.

4. COMPENSATION

The insurer shall provide compensation to the insured or nominee/legal heir(s) of the insured/beneficiary, as the case may be, as detailed below in case the insured/beneficiary suffers death/permanent total disability/permanent disability as mentioned in © and (d) below due to any reasons as mentioned in para 3 above:

(a) Death   :       Rs. 1.00 lac
(b) Permanent total disability    :      Rs. 1.00 lac
(C) Loss of two limbs, two eyes, one limb and one eye     :     Rs. 50,000/-
(d) loss of one eye or one limb    :     Rs. 25,000/-

5. PAYMENT OF COMPENSATION

The compensation shall be paid within a period of seventy two hours to the nominee (s)/ legal heir (s) case of death of the insured and to the insured in case of permanent total disability/disability due to reasons as stipulated in para 3 above or receipt of confirmation from the Sub-Divisional Officer (Civil)/ Nodal Officer appointed by government of Haryana along with the documents as mentioned in para 6 below confirming the death or permanent total disability/ disability as the case may be.

6. DOCUMENTS TO BE SUBMDITTED BY THE INSURED/NOMINEE/LEAGAL HEIR IN CASE OF CLAIM

(I) In case of Death

(a) Recommendation of Sub-Divisional Officer (Civil)/Nodal Officer;
(b) Certificate by the concerned Sarpanch or Municipal Commissioner regarding are, status of the deceased and domicile of the       deceased;
(c) Attested photocopy of voters list/ration card;
(d) Claim form;
(e) Post mortem report and death certificate issued by competent authority;
(f) Copy of first information report.

(ii) In case of permanent total disability/disability


(a) Recommendation of Sub-Divisional Officer (civil) Nodal officer;
(b) Certificate by the concerned Sarpanch or municipal commissioner regarding age, status of the insured and domicile of the insured;
(c) Medical Certificate and Medico Legal Report issued by competent authority confirming percentage and kind of disability;
(d) Attested photocopy of voters list/ ration card;
(e) Claim corm;
(f) Copy of first information report.

7. MODE FOR CLAIMING PAYMENT

(I) All the documents mentioned above shall be submitted by the insured or legal heir/ nominee of the deceased, as the case may be, to the Sub-Divisional Officer (Civil)/ Nodal Officer, who in turn shall refer all the relevant papers along with his recommendation to the insurer at the nearest Division/Branch Office of the insurer for releasing the payment as indicated above. The Sub-Divisional Officer (Civil) / Nodal Officer shall be assisted by District Social Welfare Officer and Divisional manager/Branch manager of the Insurance Company for completion of required documents for immediate release of compensation.
(ii) Compensation amount in the form of bank draft or cheque shall be prepared and submitted by the insurer within a period of seventy two hours of the receipt of required documents to the Sub-Divisional Officer (civil)/Nodal Officer who in turn shall disburse the same to the insured or legal heir/nominee of the deceased, as the case may be, under intimation to the Deputy Commissioner and Director, Department of Social justice and Empowerment, Haryana, Chandigarh.

8. INFORMATION

The insurer shall give fortnightly information to the Government (Nodal Department) and Nodal Officer in the proforma prescribed by the Government regarding claims settled and compensation disbursed under the Scheme during the course of the year. The Insurer shall disburse the amount through its field Offices. If their filed office (s) do (es) not exist at any District headquarters, then that District shall be attached with the adjacent District and such list shall be supplied to the Nodal Department within fifteen days.

9. REJECTION OF CLAIM

The insurer shall not reject any claim at its own level. In case of any doubt, the claim shall be referred to the Nodal Department i.e. Director, Social justice and Emplowerment, Haryana who shall consider the whole matter and pass a speaking order and his order shall be final.

10. MONITORING OF THE SCHEME

(I) The above scheme shall be monitored by the insurer in close association with the Department of Social justice and Empowerment, Haryana and District Administration.
(II) In case of any administrative problem, matter shall be sorted out by a committee consisting of Nodal Officer, nominated officer of Department of Social justice and Empowerment and the insurer.
You are, therefore, requested to do the needful in accordance with the above instructions and take immediate adequate steps for the implementation of the Rajive Gandhi Parivar Bima Yojna in letter and spirit as per the details given above. A copy of the claim form is also attached herewith.

11. Ladli Social Security pensions schemes

In continuation of Haryana Government Social Welfare Department notification circulated vide No.1803-SW(4)-2006 dated 18-9-2006 vide which the Ladli Social Security Scheme was launched with retrospective effect from 1st January, 2006 and further amended vide Haryana Governement Social Welfare Department notification circulated vide No. 861-SW(4)-2007 dated 4.5.2007, it is reiterated that all the conditions stipulated therein must be kept in view by the field officers while implementing the above Scheme. It should be ensured that no eligible person is left out whereas no ineligible person is given the benefit for which the applicant is not entitled under the Scheme.
All the requisite conditions which must be adhered to while dealing with the above Scheme have been serialized below for your convenience.

1. The applicant i.e. biological single parent of biological parents should have no son, biological or adopted.
2. The applicant should have only daughter/daughers. Biological adopted.
3. The benefit under the Scheme can be extended on the birth of first girl child itself if the applicant fulfills other conditions.
4. The applicant should be a domicile of Haryana.
5. The Haryana Govt. employees and their family having no son, biological or adopted, but having only daughter/daughters are also eligible under this Scheme provided they are not specifically excluded as mentioned in para-13
6. The enrolment of families under this Scheme will commerce from the 45th birthday of the mother or father whosoever is older.
7. This benefit is admissible for 15 years from 45th birthday to 60th birthday of the applicant beneficiary.
8. In case of the death of either of the parents, the surviving spouse will get this benefit i.e. Rs. 500/- per month during the period till the 60th birthday of the surviving spouse.
9. In case of single parent family the age of such parent will be the deciding factor as explained in para6. Such Single parent will get the benefit of Rs. 500/- per month from the 45th birthday to 60th birthday.
10. The Scheme is not restricted to BPL/SC/ST families only.
11. The Scheme is open for all sections of the society irrespective of the sex caste, race, creed or religion of the applicant beneficiary.
12. The following persons shall be excluded for grant of benefit under this Scheme.
1. those who themselves or their children are sales Tax Assesses.
2. Those Who themselves or their children are income Tax Assesses.
3. Those whose children are class-I/class-II Gazetted Officers in the service of the Government or hold equivalent post in public sector undertaking or are employed under a private employer and draw a salary equivalent to that of a Class-II Gazetted Officer (Rs. 12,000/- per month minimum)
4. Those whose Children are professional i.e. Doctor or lawyer or Chartered Accountant or Income Tax consultant of Dental Surgeon Engineer or Architect or Contractor etc. (This list is only illustrative and all such professionals of a similar nature may also be included.
5. Those who themselves or their children are MPs or MLAs or chairmen of Boards or corporations or have been MPs, or MLAs or chairmen of Boards or Corporations.
6. If either parent is getting old Age pensions schemes the benefit under Ladli Social Security pensions schemes can not be given to the other spouse.
7. If a son is born or adopted by the beneficiary after the sanction of the Ladli Social Security pensions schemes. the benefit of the Scheme will be stopped forth with.
